Terms & Conditions

All information obtained for diagnosis or treatment is confidential except as required or allowed by law or except to facilitate diagnosis/assessment or treatment. You will be asked to provide written authorisation for the release of any information.

I accept once a deposit or payment is made on a programme or individual, any money paid is non-refundable unless severe injury or illness happens. Proof of this will have to be given to receive any refunds.

Once a session is booked you must provide more than 12 hours notice when cancelling a session. If you miss your session or fail to provide more than 12 hours notice your payment will NOT be refunded.

I understand that if I miss sessions these cannot be carried over into future weeks unless agreed at POS with Leigh Edwards.

I understand that there are inherent risks of participating in a programme consisting of exercise. If I sustain or claim to have sustained injury whilst participating in the programme/session, I acknowledge that PHIT Sports Therapy is not responsible

I certify that questions outlined on the relevant medical forms are true and complete to the best of my knowledge. I understand and agree that it is my responsibility to inform PHIT Sports Therapy of any conditions or changes to my health, now and ongoing, which may effect my ability to participate safely and with minimal risk of injury.

Covid-19 Policy

I confirm that I;

Do not currently have COVID-19 or any symptoms of COVID-19?

Nobody in my household or support bubble has COVID-19 or symptoms of COVID-19?

Haven't been in close contact with anyone else in the past 14 days who has symptoms of COVID-19 or been contacted by the NHS Test and Trace service and told to self-isolate?

Do not have;

a high temperature – this means you feel hot to touch on your chest or back (you do not need to measure your temperature)

a new, continuous cough – this means coughing a lot for more than an hour, or 3 or more coughing episodes in 24 hours (if you usually have a cough, it may be worse than usual)

a loss or change to your sense of smell or taste – this means you've noticed you cannot smell or taste anything, or things smell or taste different to normal

If you have any of the symptoms, have been in contact with another who has symptoms documented on the policy guidelines, or feel generally unwell, it is advisable that you do NOT attend your session.

Cancellations or amendments that fall outside of the usual cancellation policy due to illness or contact with another person who is displaying symptoms or has been in contact with others who are, will be amended or refunded as necessary.

If you have also previously had a positive test for COVID-19, we would require you to obtain a letter from your GP sanctioning the treatment against the risks associated with post virus affects.

You may also be required to wear a mask during treatment sessions (especially for upper body treatments and where face to face treatment is prolonged). You can bring your own (as long as it meets requirements and is not a fashion covering) or one can be provided for you. If refusal to wear a mask, the session will be ended immediately with no refund fee.

· We will be operating on a one in, one out basis.

· Please wait inside your car or outside the clinic (at an appropriate distance for those exiting) until your specialist notifies you to come in

· Wash your hands immediately on arrival, put on a mask (provided if needed), and wash hands again prior to leaving

· Please allow us time either side of each appointment to clean and sanitise the treatment area

· You may also be required to have your temperature taken on arrival

Treatment areas will be thoroughly cleaned before and after treatments, all PPE will be disposed of accordingly.

There will be handwash stations and hand sanitiser dispensers available to use to maintain hygiene.

Please turn up on time to your appointment, too early and you will have to wait, too late and I will have to reschedule. You will receive a text when ready.
